1. Does Her Majesty the QUEEN hold any Royal Style and Titles, as held to Royal Style and Titles Act 114 of 1973 of:-
ELIZABETH THE SECOND, by the Grace of God, Queen of Australia and Her other Realms and Territories, Head of the Commonwealth.

2. Is Her Majesty the QUEEN, known by any other Royal Style and Title/s as ELIZABETH II.
Answer Yes/No.
3. Is Her Majesty the QUEEN the Supreme Commander – or holding any such title of the Anglican Church of Australia.

4. Is Her Majesty the Queen the Supreme Commander – or holding any such title of the Church of England.
